The Oregon Court Visitor Report (Adult Guardianship) is a document created to provide an independent evaluation of an individual's suitability for guardianship. It is completed by an independent and impartial court-appointed visitor, who evaluates the individual's mental and physical capacity, whether there is a need for guardianship, and whether the proposed guardian is suitable. The report is filed with the court and can be requested by any interested party. There are three types of Oregon Court Visitor Reports: the Initial Report, the Annual Report, and the Termination Report. The Initial Report includes a comprehensive evaluation of the individual, including the visitor's opinion about the suitability of guardianship, and a recommendation for the type and scope of guardianship. The Annual Report provides a review of the guardian's performance and an assessment of the individual's continued need for guardianship. The Termination Report provides an evaluation of the guardian's discharge of their duties and makes a recommendation regarding the termination of the guardianship.
