Oregon Notice To Elderly Person-Person With Disabilities-Objections And Request for Hearing (Sweepstakes) is a standardized legal form developed by the Oregon State Bar for elderly and disabled persons to file an objection or request a hearing regarding a sweepstakes or other similar promotion. This form must be filled out completely and accurately by the elderly or disabled person, and must be signed and dated before filing.
