The Oregon Affidavit In Support of Indigent Guardianship Fund Application And Or Motion To Waive Fees And Costs is an official document that is used to apply for and/or waive fees and costs associated with guardianship proceedings in the state of Oregon. The affidavit is made under penalty of perjury and must be filled out completely and accurately in order to be accepted. It is signed by the applicant and must be notarized.
