The Oregon EFS-3a Addendum (Must always accompany an EFS-3) is an addendum that must accompany an Employment-Related Expense Form (EFS-3) when submitting a claim for reimbursement of employment-related expenses to the State of Oregon. The addendum contains additional information that must be included in the claim. It is divided into three sections: 1. Expense Description: This section requires the claimant to provide a detailed description of the expense, including who incurred the expense, when and where it was incurred, the purpose of the expense, and the payment method used. 2. Expense Certification: This section requires the claimant to certify that the expense was actually incurred, that all information provided is true and accurate, and that the claimant is not seeking reimbursement for an expense already paid by or on behalf of the claimant. 3. Business Activity: This section requires the claimant to provide additional information regarding the business activity related to the expense, including the type of activity, the date of the activity, the location of the activity, and the name of the person or organization for whom the activity was performed.
