An Oregon Affidavit of Service is a legal document that is used to verify that legal documents have been properly served to the intended party. It is a sworn statement made by the person who served the documents, stating the date, time, and manner in which the documents were served. There are two types of Oregon Affidavit of Service: personal service and substituted service. Personal service is when the documents are delivered directly to the person or served by leaving it with someone of suitable age and discretion at their residence or place of employment. Substituted service is when the documents are sent by registered mail, or left with someone of suitable age and discretion at the business or residence of the person to be served. In either case, the Affidavit of Service must be completed and signed by the server and notarized.
