The Oregon Confidential Information Form (CIF) is a document issued by the Oregon Department of Human Services (DHS) to allow the protection of confidential information related to individuals’ health, safety, or financial information. The CIF is used to provide the necessary safeguards to ensure that individuals’ sensitive information is properly secured and not released to unauthorized parties. It is also used to ensure that the information is not used for any purpose other than the authorized purpose. There are two types of Oregon Confidential Information Forms (CIF's): the DHS CIF and the county CIF. The DHS CIF is used to protect confidential information related to DHS programs, services, or activities. The county CIF is used to protect confidential information related to county programs, services, or activities. Both forms require the individual to provide identifying information, such as name, address, and contact information, and signature.
