Oregon Notice To Respondent — Request For Hearing (Sweepstakes) is a document issued by the Oregon Lottery to a Respondent who has been entered into a sweepstake. This document informs the Respondent that the Oregon Lottery has received an entry from them and that they are being requested to appear for a hearing. The document includes the date, time, and location of the hearing, as well as a list of documents the Respondent must bring to the hearing. This document is issued in three different types: 1) Sweepstakes Request for Hearing; 2) Sweepstakes Dispute Hearing Request; and 3) Sweepstakes Appeals Hearing Request. The first type is issued when a Respondent has submitted a winning ticket or entry to the Oregon Lottery and wants to dispute the outcome or prize. The second type is issued when a Respondent has an issue with the Oregon Lottery that needs to be resolved. The third type is issued when a Respondent wants to appeal a decision made by the Oregon Lottery.
