The Oregon Affidavit of Service (Alternative Service) (6A) is a legal document used to provide proof of service of legal documents. It is used when service of process is not possible in the traditional manner. The alternative service methods allowed under Oregon law are mail, publication, or posting. The affidavit must be completed and signed by the person who performed the alternative service, and it must include the date, time, and location of the service, as well as the name of the person served. There are two types of Oregon Affidavit of Service (Alternative Service) (6A): an Affidavit of Service by Mail, and an Affidavit of Service by Publication or Posting. An Affidavit of Service by Mail is used when the documents were sent via registered or certified mail to the last known address of the person to be served. An Affidavit of Service by Publication or Posting is used when the documents were published in a newspaper or posted on the courthouse door.
