The Oregon Uniform Support Declaration (6F) can include the non-custodial parent's name, address, Social Security Number, and income, as well as details regarding child support and medical support payments. There are three types of the Oregon Uniform Support Declaration (6F): Administrative Order, Stipulated Agreement, and Judgement. The Administrative Order is issued by the court upon request; the Stipulated Agreement is a mutual agreement between the parties; and the Judgement is a court order entered by the court.
