Oregon Pharmaceutical Clinical Justification for Workers' Compensation is a program designed to ensure that workers' compensation claimants receive medically necessary prescription drugs and medications in a cost-effective manner. This program requires health care providers to submit a clinical justification form to the Oregon Workers' Compensation Division (CD) that outlines the medical necessity of the prescribed drug and includes information such as the diagnosis, reason for the drug, and expected outcomes of the treatment. Depending on the type of drug prescribed, the CD may require additional information from the provider, such as a comparison of the costs of alternative medications or additional clinical justification. There are three types of Oregon Pharmaceutical Clinical Justification for Workers' Compensation: 1. Initial Authorization: This is the initial request for authorization of a prescription drug or medication. 2. Subsequent Authorization: This is a request for additional authorization of a prescription drug or medication that has previously been authorized. 3. Excess Authorization: This is a request for authorization of a prescription drug or medication that exceeds the quantity or dosage that was previously authorized.
