The Pennsylvania Non-Renewal Notice Form is a document used to notify an insurance company that the policyholder does not wish to renew their insurance policy. This document is issued by the policyholder and informs the insurance company of their decision to not renew the policy. There are two types of Pennsylvania Non-Renewal Notice Form: the Notice of Non-Renewal of an Individual Policy and the Notice of Non-Renewal of an Unincorporated Business. The Notice of Non-Renewal of an Individual Policy is used when the policyholder wishes to cancel an individual insurance policy. The Notice of Non-Renewal of an Unincorporated Business is used when the policyholder wishes to cancel an insurance policy taken out on behalf of an unincorporated business. Both forms must be completed and signed by the policyholder, and sent to the insurance company at least 90 days before the policy's expiration date.
