A Pennsylvania Statement of Correction is a legal document used by Pennsylvania residents to correct or amend information on a birth or death certificate. It is typically filed with the Pennsylvania Department of Health's Division of Vital Records. There are three types of Pennsylvania Statement of Correction: a Correction Statement, an Amendment Statement, and an Amended Birth Certificate. A Correction Statement is used to change information on a birth or death certificate, such as the child's name, date of birth, or place of birth. An Amendment Statement is used to add information to a birth or death certificate, such as the father's name or the maiden name of the mother. An Amended Birth Certificate is used to update a birth certificate when changes have been made to the birth record. The Pennsylvania Statement of Correction must be completed using the appropriate form provided by the Pennsylvania Department of Health and must be signed by the person submitting the correction, as well as the custodian of the original birth or death certificate. The form must also be accompanied by any supporting documentation, such as a court order or proof of the correct information. Once the statement is accepted by the Department of Health, a new birth or death certificate will be issued reflecting the changes.
