The Pennsylvania Statement of Domestication [DSC:15-375] is a form used by businesses that are registered in other states, but wish to become registered to do business in the Commonwealth of Pennsylvania. This form must be filed with the Pennsylvania Department of State Bureau of Corporations and Charitable Organizations (DSC). There are two types of forms available: the Standard and Short Form. The Standard Form is used by corporations, limited liability companies, and limited partnerships that are registered in other states and wish to be registered to do business in Pennsylvania. This form requires the company to provide information such as the name of the business entity, its state of incorporation or formation, its date of incorporation or formation, the name and address of its registered agent, and the business activity of the entity. The Short Form is used by limited liability companies that are already registered in other states and wish to register to do business in Pennsylvania. This form requires the company to provide the information mentioned above, as well as a statement that the company is in compliance with the requirements of the Pennsylvania Limited Liability Company Law. Both forms must be signed by an authorized representative of the business entity and filed with the Department of State.
