The Pennsylvania Statement of Merger [DSC:15-335] is a document required by the Department of State of Pennsylvania when two or more corporations merge into one. This document is filed with the Pennsylvania Department of State Bureau of Corporations and Charitable Organizations (DSC) and is used to register the merger of the entities. The Statement of Merger must include the name and address of each of the merging entities, the date of the merger, the date the Plan of Merger was approved, the name and address of the surviving entity, the date the surviving entity was incorporated, the name and address of the registered agent of the surviving entity, and the plan of merger, if any.
