The Pennsylvania Certificate of Service (Administrative Procedures — Form C) is a form used by the Commonwealth of Pennsylvania to certify that certain documents have been served upon the required parties. This form is required for service of process in all administrative proceedings governed by the Pennsylvania Administrative Code. The form includes sections for the type of service, the name and address of the service recipient, and the date of service. It must be signed by the person who served the documents. There are three types of Pennsylvania Certificate of Service (Administrative Procedures — Form C): personal service, first-class mail service, and certified mail service. Personal service requires that the recipient be physically served with documents, first-class mail service requires the documents to be sent via first-class mail, and certified mail service requires the documents to be sent via certified mail.
