Pennsylvania Form AO 213 Vendor Information/Tin Certification (Please also see Notice regarding Arbitration Payment.) is a form required by the Pennsylvania Department of Revenue for vendors who submit a Sales Tax report. This form is used to certify that the taxpayer identification number (TIN) provided by the vendor is correct. The TIN must be provided to the Department in order to properly report and pay sales tax. The form also provides information on arbitration payments, which are paid out when an arbitration dispute is resolved in favor of the vendor. There are two types of Pennsylvania Form AO 213 Vendor Information/Tin Certification (Please also see Notice regarding Arbitration Payment.): Form AO 213-A, which is for vendors who are not registered with the Department of Revenue for sales tax purposes, and Form AO 213-B, which is for vendors who are registered with the Department of Revenue for sales tax purposes.
