Pennsylvania Local Bankruptcy Form 1002-2A, Statement of Qualification As Complex Chapter 11 Case, is a bankruptcy form that must be filed by a debtor, or an entity that is filing a bankruptcy petition under Chapter 11 of the United States Bankruptcy Code, in the Middle District of Pennsylvania. This form must be filed in order to qualify as a complex chapter 11 case. The Pennsylvania Local Bankruptcy Form 1002-2A consists of two parts: the Statement of Qualification and the Statement of Intent. The Statement of Qualification requires the debtor to provide detailed information about their assets, debts, and other financial matters. The Statement of Intent requires the debtor to state their intent to reorganize and propose a plan to the court for how they will pay off their creditors. There are two types of Pennsylvania Local Bankruptcy Form 1002-2A, Statement of Qualification As Complex Chapter 11 Case: the initial filing and the amended filing. The initial filing must be made within 45 days of the filing of the bankruptcy petition. The amended filing must be made within 90 days of the filing of the bankruptcy petition.
