Pennsylvania Local Bankruptcy Form 2016-2A, "Short Form" Application of Counsel for Debtor for Compensation and Reimbursement of Expenses in Chapter 13 Case Pursuant to L.B.R. 2016-2(a) is a form used by attorneys to apply for compensation and reimbursement of expenses in Chapter 13 cases in Pennsylvania. It is used to request an allowance of attorney’s fees and other expenses for representing the debtor. The form is divided into two parts: (1) Short Form, and (2) Long Form. The Short Form is a simple, one-page application that lists the name of the debtor, the debtor’s counsel, the date the application was filed, and the amount requested. The request must be supported by an affidavit from the debtor's attorney attesting to the reasonableness of the requested amount. The Long Form is a more detailed four-page application which includes a breakdown of the services rendered by the attorney and the corresponding fees, as well as a detailed description of the expenses that the attorney incurred in representing the debtor. This form must also be supported by an affidavit and must be accompanied by itemized billing statements and other documentation showing the services performed and the expenses incurred. Both forms must be filed with the court in order for the attorney to be compensated for services rendered and expenses incurred in representing the debtor in a Chapter 13 case.
