Joint Case Management Plan
The Pennsylvania Joint Case Management Plan (CM) is a collaborative effort between the Department of Human Services (DHS), the Office of Children, Youth and Families (CF), and the Office of Mental Health and Substance Abuse Services (MESAS). The CM is designed to ensure that children, youth, and families in need of behavioral health services receive access to a comprehensive array of coordinated services in a timely manner. The CM consists of four main components: Case Management Services, Coordinated Service Delivery, Quality Management, and Utilization Review. Case Management Services provide assessment, planning and coordination of services for children, youth, and families in need of behavioral health services. Coordinated Service Delivery involves collaboration between the DHS, CF, and MESAS to ensure a comprehensive array of services is available. Quality Management includes developing and monitoring quality standards and performance improvement activities. Utilization Review involves monitoring the appropriate and effective utilization of services. Pennsylvania has two types of Joint Case Management Plans: the Standard Joint Case Management Plan and the Enhanced Joint Case Management Plan. The Standard CM is available to children and youth who are receiving services through a single agency. The Enhanced CM is available to children, youth, and families who are receiving services from multiple agencies. Both plans provide case management services, coordinated service delivery, quality management, and utilization review.
