The Pennsylvania Civil Cover Sheet (JS 44) is a form used by the courts in the state of Pennsylvania to provide the court with information about the parties involved in a civil action and the nature of the case. It is a one-page document that must be completed and filed with the court at the time the complaint is filed. The Pennsylvania Civil Cover Sheet (JS 44) includes information such as the names and addresses of the parties, the name and address of their attorneys, the type of action being filed, the relief sought, the amount of damages claimed, and the jurisdiction of the court. There are three types of Pennsylvania Civil Cover Sheet (JS 44): JS 44A, JS 44B, and JS 44C. JS 44A is used for cases such as small claims, equity, and landlord/tenant; JS 44B is used for cases such as appeals, writs, and mandamus; and JS 44C is used for all other civil cases.
