The Pennsylvania Notice of Appeal From Summary Criminal Conviction is a document used by an individual to appeal a summary criminal conviction in the Commonwealth of Pennsylvania. This document is filed with the court in the county where the conviction occurred, and is used to formally request an appeal of the summary conviction. There are two types of Pennsylvania Notice of Appeal From Summary Criminal Conviction: Notice of Appeal from Summary Criminal Conviction and Notice of Appeal from Summary Criminal Conviction pursuant to the Post Conviction Relief Act. The Notice of Appeal from Summary Criminal Conviction is used to appeal a summary conviction in which the sentence has already been served. The Notice of Appeal from Summary Criminal Conviction pursuant to the Post Conviction Relief Act is used to appeal a summary conviction in which the sentence has not yet been served. Both notices must include the name of the defendant, the court and county where the conviction was made, the date of the conviction, and the offense for which the conviction was made. The notice must then state the grounds for the appeal and any other relevant information. The notice must be signed by the defendant, and a copy must be sent to the district attorney's office.
