The Pennsylvania Family Court Pro Se Entry of Appearance is a form that must be filled out by individuals who are representing themselves (pro SE) in a family court case, without an attorney. This document serves as an official notice to the court that the individual is representing themselves in a legal proceeding. It also includes information about the individual’s rights and responsibilities in the case. There are two types of Pennsylvania Family Court Pro Se Entry of Appearance: one for custodial parents and one for non-custodial parents. Both forms require the individual to provide their name, address, phone number, and the name of the case. In addition, each form requires the individual to check boxes indicating whether the case is being heard in a county court or a district court. The custodial parent form requires the individual to provide information about the other parent such as their name, address, and phone number, as well as whether they are represented by an attorney. The non-custodial parent form requires the individual to provide the name and address of the custodial parent. Once completed, the Pennsylvania Family Court Pro Se Entry of Appearance must be signed and dated in the presence of a notary public. It must then be filed with the court in order for the case to proceed.
