The Pennsylvania Certificate of Service (Instructions and Form) is a document used by attorneys in the state of Pennsylvania to certify that service of legal documents has been made upon parties in a particular case. It is typically included with the documents being served upon the parties, and must be completed and signed by the attorney or entity responsible for making the service. There are two main types of Pennsylvania Certificate of Service: the "Instructions and Form" and the "Instructions Only". The "Instructions and Form" is a single document which contains both the instructions on how to complete the form, and the form itself. The "Instructions Only" is a separate document from the form, and contains only the instructions for completing the form. Both versions of the Certificate of Service must include the name of the party to whom service was made, the date of service, the name of the person making the service, and the method of service (either personal or mail). Additionally, the form must be signed by the attorney responsible for making the service.
