The Pennsylvania Guardian of the Person Annual Report (for an Incapacitated Person) is an official document that is required by the state of Pennsylvania to be completed by a guardian of an incapacitated person. The report is filed annually and is used to provide the court with information about the incapacitated person’s current care and services, as well as any changes in guardianship and/or care since the last report. The report is also used to document the guardian’s compliance with the Pennsylvania Guardianship Law. The report includes information about the incapacitated person’s living arrangements, finances, medical care, and other services. The two types of Pennsylvania Guardian of the Person Annual Report for an Incapacitated Person are the Initial Report and the Annual Report. The Initial Report must be filed within 90 days of appointment of the guardian, and it includes information about the incapacitated person’s background, living arrangements, financial records, and other services. The Annual Report must be filed within 90 days of the anniversary of the guardian’s appointment. The Annual Report includes information about the guardian’s compliance with the Pennsylvania Guardianship Law and any changes in the incapacitated person’s care and services since the last report.
