Pennsylvania Motion Cover Sheet and Notice (Family-Orphans Court) is a document used in family law proceedings in the Commonwealth of Pennsylvania. This cover sheet and notice is used to inform the court of the filing of a motion or petition and to provide a summary of the relevant information in the motion. It is also used to provide notice to all parties of the filing, to set a date and time for the hearing, and to provide instructions for filing a response to the motion. There are two types of Pennsylvania Motion Cover Sheet and Notice (Family-Orphans Court); one for motions and one for petitions. The motion cover sheet and notice includes information such as the title of the motion, the court in which the motion is filed, the names of the parties involved, a description of the relief sought, the date of the hearing, and the names of any witnesses who may be called to testify. The petition cover sheet and notice includes the same information in addition to the title of the petition, a description of the relief requested, and the date of filing. Both cover sheets and notices are required to be filed with the court in order for the motion or petition to be heard.
