Pennsylvania Name Change Procedure is the process of legally changing one’s name in the state of Pennsylvania. There are two types of Pennsylvania Name Change Procedure: adult name changes and minor (under 18) name changes. For adult name changes, the petitioner must first complete a Name Change Petition Form, which is available online at the Pennsylvania Department of State website. The form must include the petitioner’s current name, the desired name, and the petitioner’s address. The petitioner must then submit the form, along with the applicable name change fee, to the Prothonotary’s office in the county where they reside. Once the form is approved, the petitioner must publish a Notice of Intention to Change Name in two county newspapers for two consecutive weeks. After the notice has been published, the petitioner must return to the Prothonotary’s office to sign an Oath of Publication and receive a court order granting the name change. For minor name changes, the petitioner must complete a Minor Name Change Petition Form. The form must include the name of the minor, the minorbirthdatete, the minor’s current name, the desired name, and the petitioner’s address. The form must be signed by both parents or legal guardians of the minor. The petitioner must then submit the form, along with the applicable name change fee, to the Prothonotary’s office in the county where they reside. Once the form is approved, the petitioner must publish a Notice of Intention to Change Name in two county newspapers for two consecutive weeks.
