The Pennsylvania Entry of Appearance is a document filed with the court that formally indicates an attorney’s representation of a party in a case. The document informs the court that the attorney is authorized to practice law in the state of Pennsylvania and is authorized to represent the party in the case. Depending on the nature of the case, there are two types of Entry of Appearance forms: the Entry of Appearance for an Attorney Representing a Party and the Entry of Appearance for an Attorney Appearing Pro HAC Vice. The Entry of Appearance for an Attorney Representing a Party is used when an attorney is representing a party in a case within the state of Pennsylvania. The attorney must sign the form and provide their contact information, as well as the contact information for the party they are representing. The Entry of Appearance for an Attorney Appearing Pro HAC Vice is used when an attorney is representing a party in a case outside the state of Pennsylvania and is requesting to appear in the case before the Pennsylvania court. The attorney must provide proof that they are licensed to practice law in another state, a signed affidavit from a Pennsylvania attorney who is willing to be responsible for the out-of-state attorney’s actions in the case, and a completed Entry of Appearance form.
