The Pennsylvania New Statewide Civil Cover Sheet (149.0 KiB) is a document used in Pennsylvania courts when filing a civil action in state court. This comprehensive document helps organize a broad range of information, including the parties involved, the type of action, and the relief sought. The cover sheet also assists the court in assigning the case to the proper judge and division. There are three types of Pennsylvania New Statewide Civil Cover Sheet: a General Cover Sheet, a Cover Sheet for Appeal, and a Cover Sheet for Complaint in Equity. The General Cover Sheet is used for all new civil actions, except for those that fall under the jurisdiction of the complex litigation center. The Cover Sheet for Appeal is used when filing an appeal in the Pennsylvania Supreme Court. The Cover Sheet for Complaint in Equity is used when filing a complaint in equity, such as a complaint for the appointment of a receiver or a complaint to quiet title.
