The Pennsylvania Boards, Authorities, and Commissions Application (748.0 KiB) is a necessary document for individuals who wish to become members of a Pennsylvania board, commission, or authority. The application provides information about the applicant's qualifications, experience, and training. It also outlines the roles and responsibilities of the applicant in the board, commission or authority. There are two types of Pennsylvania Boards, Authorities, and Commissions Applications: 1) an application for a board, authority, or commission established by the Pennsylvania General Assembly; and 2) an application for a board, authority, or commission established by a municipality or county. Both applications require the applicant to provide information such as their name, address, and contact information, a resume, and a statement of interest in the board, commission, or authority.
