The Pennsylvania Right to Know Request Form (71.9 KiB) is a standard form used by individuals in the state of Pennsylvania to request access to public records. This form is used to make a formal request to a government agency, such as a state agency, county agency, or school district, for access to public records. The form includes fields to list the requester's name, address, phone number, email address, and the records being requested. It also includes a box to check if the requester is making the request in the name of an organization, and a box to check if the request is being made for commercial purposes. There are two versions of the Pennsylvania Right to Know Request Form (71.9 KiB): the Standard Request Form, and the Expedited Request Form. The Standard Request Form is used to make a regular request for records, while the Expedited Request Form is used to make an emergency request for records.
