The Pennsylvania Change of Name Petition (305.9 KiB) is a legal form that allows individuals to change their name in the Commonwealth of Pennsylvania. This form is used when an individual wishes to change their name for any reason, such as marriage, adoption, religious conversion, or even to simply change their name.
