The Pennsylvania Income Survey Form (105.8 KiB) is a document used by the Pennsylvania Department of Revenue to collect information on the income of Pennsylvania residents. The form is divided into two sections: a Form 1040 and a Schedule C. The Form 1040 is used to report an individual's income from wages, salaries, tips, and other sources, while the Schedule C is used to report income from self-employment, rental properties, and other business activities. The form also allows for deductions and credits to reduce the amount of taxes owed. There are two different types of the Pennsylvania Income Survey Form (105.8 KiB): one for individual taxpayers and one for businesses. The individual taxpayer version asks for an individual's name, Social Security number, and address, as well as income information. The business version asks for the business name, address, and entity type, as well as income information. Both versions of the form require the taxpayer to attach supporting documents such as W-2 forms and 1099 forms.
