Pennsylvania Write-Ins 5-16-17 Judge & Inspector (33.3 KiB) is a form used to select write-in candidates for judge or inspector positions in Pennsylvania in the 2017 Primary and General elections. This form is used to appoint electors to serve as write-in judge or inspector for any given election. It also provides information regarding the filing requirements for write-in candidates in Pennsylvania. The form consists of two parts: the first part is a Declaration of Write-In Candidacy and the second part is a Certificate of Election. The Declaration of Write-In Candidacy requires the candidate to provide information including name, address, party affiliation, and date of birth. The Certificate of Election requires the candidate to provide the name and address of the write-in judge or inspector, as well as the date of the election. This form is available in both English and Spanish versions.
