Pennsylvania Name Change Petition is a legal process for individuals to apply for a change of name in the state of Pennsylvania. This process is governed by the Pennsylvania Rules of Civil Procedure 1501-1511 and allows an individual to change their name in order to reflect a new identity or to correct a mistake on a birth certificate. There are two types of Pennsylvania Name Change Petitions: Adult Name Change Petition and Minor Name Change Petition. An Adult Name Change Petition is used when a person over the age of 18 seeks a name change. The petitioner must provide a valid driver’s license or state-issued identification card, certified copies of birth and marriage certificates, and proof of Pennsylvania residency. The Minor Name Change Petition is used when a person under the age of 18 seeks a name change. The petitioner must provide a certified copy of the minor’s birth certificate, consent of both parents, and proof of Pennsylvania residency. After the petition is filed and the court grants the request, a new birth certificate and other documents will be issued reflecting the new name.
