The Pennsylvania Mental Health Procedures Act (MPA) Cover Sheet is a document that must accompany certain mental health care requests submitted to the State of Pennsylvania. This form is used to provide accurate and comprehensive information about a particular request, as well as to certify that the request is in compliance with the MPA. The Cover Sheet is also used to provide the Pennsylvania Department of Human Services with the necessary information to process requests for mental health services. There are two types of Pennsylvania Mental Health Procedures Act Cover Sheet. The first is the Certification Cover Sheet, which is used when a person or their representative is making a request for mental health services. This Cover Sheet must be completed and signed by the requester and their representative, if applicable. The second type of Pennsylvania Mental Health Procedures Act Cover Sheet is the Premiering Cover Sheet. This Cover Sheet must be completed and signed by the requester and their representative, if applicable, prior to the pre-hearing conference. The information provided on the Premiering Cover Sheet will be used to determine the requester's eligibility for mental health services, and to determine the nature and extent of the services to be provided.
