Pennsylvania Rule 801 Verification of Certification — Form 01-207 is a form that is used to verify the certification of an individual or entity in the state of Pennsylvania. It is used to confirm the accuracy and validity of information that has been submitted to the Pennsylvania Department of State. The form contains six sections: 1) Information about the Certifying Entity; 2) Information about the Person or Entity Being Certified; 3) Information about the Certification; 4) Authorization for Release of Information; 5) Certification Statement; and 6) Signature of Certifying Entity. The two types of forms are the individual version and the entity version.
