The Pennsylvania Minors and Incapacitated Persons Checklist is a tool used to help determine if a minor or incapacitated person has a guardian or conservator and if an appointment is necessary. This checklist, also known as the PA MIP Checklist, covers various categories of guardianship and conservatorship and includes questions regarding the minor’s or incapacitated person’s physical and mental capacity, financial and legal responsibilities, and lifestyle. There are three types of Pennsylvania Minors and Incapacitated Persons Checklists: the Standard Checklist, the Mental Health Checklist, and the Developmental Disability Checklist. The Standard Checklist is used for minors and incapacitated persons who do not have a mental health diagnosis and/or developmental disability. The Mental Health Checklist is used for minors and incapacitated persons who have a mental health diagnosis and/or developmental disability. The Developmental Disability Checklist is used for minors and incapacitated persons who have a developmental disability.
