The Pennsylvania Return of Service Affidavit is a legal document required by the Commonwealth of Pennsylvania. It is an affidavit that is submitted to the court to show proof of service, or delivery, of legal documents to the other party in a court action. The affidavit is signed by the individual who served the documents, known as the Server, and notarized by a public notary. There are two types of Pennsylvania Return of Service Affidavit: * The Personal Service Affidavit is used when the documents are served in person to the other party. This affidavit must include the date, time, and place of service, the method of service, the name of the person served, and the name of the Server. * The Substituted Service Affidavit is used when the documents are served by mail, publication, or by another method authorized by the court. This affidavit must include the date, time, and type of service, the name of the person served, the name of the individual who accepted service on behalf of the other party, and the name of the Server. The Pennsylvania Return of Service Affidavit must be filed with the court in order to provide proof that the other party has been properly served with the legal documents.
