The Pennsylvania Government Access Request Form (To be used by Governmental Entities) is a form used by government entities in the state of Pennsylvania to request access to the government’s records. The form is designed to ensure that government entities can efficiently and securely access records that are otherwise unavailable to the public. The form includes information about the requesting entity, the records being requested, and the purpose of the request. Depending on the type of records being requested, additional information may be required. The Pennsylvania Government Access Request Form (To be used by Governmental Entities) is divided into two types: Non-Personal Records Request Form and Personal Records Request Form. The Non-Personal Records Request Form is used to request access to records that do not contain information about individuals. This form must be completed and submitted to the Pennsylvania Department of State by the requesting entity. The Personal Records Request Form is used to request access to records that contain information about individuals. This form must be completed and submitted to the Pennsylvania Department of State by the requesting entity as well as the individual whose records are being requested. Once the form has been completed and submitted, the Pennsylvania Department of State will review the request and approve or deny access to the requested records.
