The Pennsylvania PAS CDU Direct Deposit Enrollment Form is a document used to authorize direct deposit of payments into a bank account. It is a secure way to receive payments from the Commonwealth of Pennsylvania. The form is used to provide the Commonwealth with the necessary information to set up direct deposit for payments like refunds, child support, tax refunds, pensions, unemployment, and workers’ compensation. Pennsylvania offers two types of direct deposit enrollment forms: 1) the Statewide Centralized Direct Deposit (CDU) form and 2) the Common Payment Enrollment (CPE) form. The CDU form is used to enroll in direct deposit for payments issued by the Department of Public Welfare, the Department of Labor and Industry, and the Department of Education. The CPE form is used to enroll in direct deposit for payments issued by all other state agencies. The form requires the enrolled to provide personal information, including financial institution details and account information.
