The Pennsylvania Mental Health Notice is a form issued by the Pennsylvania Department of Human Services that provides individuals, guardians, and/or representatives of individuals with mental health issues with vital information about their rights and responsibilities when receiving mental health services in Pennsylvania. This document outlines the rights that individuals have in relation to their care, and provides information on how to access and receive mental health services in Pennsylvania. It also outlines the responsibilities of providers of mental health services in Pennsylvania. There are two types of Pennsylvania Mental Health Notice: the Adult Mental Health Notice and the Child Mental Health Notice. The Adult Mental Health Notice is intended for individuals 18 years of age and older, while the Child Mental Health Notice is intended for individuals under 18 years of age.
