Pennsylvania REV-487 -- Entry Into Safe Deposit Box to Remove A Will or Cemetery Deed is a form used to gain access to a safe deposit box in Pennsylvania to remove a will or cemetery deed. This form is used when the box holder is deceased and there are no other heirs or executors to the estate. The form must be signed by a legal representative of the estate, such as an executor, administrator, or court-appointed guardian. It must also be notarized and witnessed by two people, one of whom must be the box holder’s surviving spouse, if applicable. The form is divided into two parts. The first part is the application for entry into the safe deposit box. This section requires the name and address of the person requesting entry, the name of the deceased box holder, and the location of the box. The second part of the form is the affidavit of the person requesting entry. This part requires the person to swear that they are the legal representative of the estate and that they will only use the contents of the box to fulfill the estate’s obligations.
