This survey is one of only a few surveys for which all recipients are required by law to respond. The U.S. Census Bureau is required by law to protect your information. If you need help or have questions about completing this form, please call 1-800-354-7271.

Those who decline to complete the survey may receive visits to their homes from Census Bureau personnel. Because it is a mandatory survey, it is governed by federal laws that could impose a fine of as much as $5,000 on those who flagrantly refuse to participate.

If no response is returned ? or if the answers are incomplete ? the Census Bureau will follow up to get the information. The final population tally is used to divvy up more than $675 billion annually to states for all sorts of programs. By census law, refusal to answer all or part of the census carries a $100 fine.
