The Rhode Island Statement of Change of Agent is a form used to change the registered agent for a business entity registered with the Rhode Island Secretary of State. This form is essentially a statement that informs the Secretary of State that the registered agent for the business entity has been changed. There are two types of Rhode Island Statement of Change of Agent: Statement of Change of Agent for a Domestic Business Entity and Statement of Change of Agent for a Foreign Business Entity. The form must be signed by an officer or other authorized signer of the business entity and must list the name and address of the new registered agent. The form must also include the Rhode Island File Number and the name of the business entity. Once the form has been completed and signed, it must be filed with the Rhode Island Secretary of State. Upon receipt of the form, the Secretary of State will update the business entity's records with the new registered agent information.
