South Carolina Employers Notice of Claim and Or Request For Hearing is a document used by employers in South Carolina to file a claim for workers' compensation benefits. The notice must be filed with the South Carolina Workers' Compensation Commission, along with supporting documentation, in order to initiate the process of receiving benefits. The notice must include the name of the employer, the employee's name and address, the date of injury, the type of injury, and the name of the insurance carrier, if applicable. There are two types of South Carolina Employers Notice of Claim and Or Request For Hearing documents: the Notice of Claim and the Request for Hearing. The Notice of Claim is used by employers to provide the necessary information to the Commission in order to initiate the process of receiving workers' compensation benefits. The Request for Hearing is used by employers to request a hearing to dispute a decision by the Commission regarding their claim.
