The Tennessee Civil Cover Sheet is a form that must be completed and filed with the Clerk of Court at the time of filing a new civil lawsuit. There are three types of Tennessee Civil Cover Sheet: general cover sheet, summary process cover sheet, and small claims cover sheet. The general cover sheet is used for all lawsuits not involving summary process or small claims. The summary process cover sheet is used for cases involving eviction or illegal detained. The small claims cover sheet is used for small claims cases.
