Tennessee GUARDIAN — Sworn Examination Report is a form used to document an employee’s background investigation results when applying for positions within the state of Tennessee. The report is completed by a sworn law enforcement officer employed by the state of Tennessee’s Department of Safety and Homeland Security. It includes the applicant’s full name, address, date of birth, social security number, education, and employment history. The report also includes a summary of the investigation, which includes criminal history, as well as a photograph and signature of the applicant. The report is typically used for positions within law enforcement, corrections, and other public safety positions. There are three types of Tennessee GUARDIAN — Sworn Examination Reports: Law Enforcement Sworn Examination Report, Corrections Officer Sworn Examination Report, and Public Safety Sworn Examination Report.
