The Tennessee Statement as to Filing of the USA or TN Income Tax Return is a document used to report whether an individual has filed their federal or Tennessee state income tax return. It is issued by the Tennessee Department of Revenue and must be filed with the appropriate local or state tax authority. There are three types of Tennessee Statement as to Filing of the USA or TN Income Tax Return: Form T-5, Form T-6, and Form T-7. Form T-5 is used to report whether the taxpayer has filed their federal income tax return. It must be signed by the taxpayer and mailed to the Tennessee Department of Revenue. Form T-6 is used to report whether the taxpayer has filed their Tennessee state income tax return. It must be signed by the taxpayer and mailed to the Tennessee Department of Revenue. Form T-7 is used to report whether the taxpayer has filed both their federal and Tennessee state income tax returns. It must be signed by the taxpayer and mailed to the Tennessee Department of Revenue.
