The Tennessee Application for Cancellation of Assumed Name is a legally binding document used to declare the dissolution of a fictitious business name that was registered with the Tennessee Secretary of State. There are two types of Tennessee Application for Cancellation of Assumed Name: Form SS-4136 for businesses that filed their assumed name certificate with the Tennessee Secretary of State, and Form SS-4137 for businesses that filed their assumed name certificate with a county clerk. Both forms require the assumed name, principal place of business, and address of the business to be submitted. Additionally, Form SS-4136 requires the date the assumed name was registered with the Tennessee Secretary of State, while Form SS-4137 requires the county in which the assumed name was registered. Both forms must also be signed and notarized by the business owner.
