The Texas Data Sheet for Occupational Driver is a document used by the Texas Department of Public Safety (DPS) to assess the qualifications of a driver who is applying for an occupational driver’s license. The Texas Data Sheet includes information such as the driver’s name, date of birth, Social Security number, driver’s license number, date of issuance, and expiration date. It also includes any applicable endorsements, such as those for hazardous material, passenger or school bus, or commercial vehicle operation. The data sheet is also used to show any traffic violations, accidents, or other driving infractions. There are two types of Texas Data Sheet for Occupational Driver. The first type is the standardized form used by all Texas DPS offices. This form is available online and is used for all drivers applying for an occupational driver’s license. The second type is the customized form that is used by the specific DPS office that the driver is applying to. This form includes additional information specific to that office.
